You can make a call to the number to have the mail delivered again, but it is a bit troublesome if you don't know what the machine voice says. So it's better to go to JapanPost website on your computer or smartphone.

Most of what you need to do is to refer to what is marked on the notice you have and choose the same things on the form.
Sections with this mark in red 必須 (ひっす) are what you must fill in, so do not forget!
次に進む (つぎにすすむ) in red means Go next.
Hair Donation in Japan
Months ago I had a self-haircut at home. I wanted to use my hair for people in need.
I sent my hair to NPO "JHD&C"who handles hair donation from all over Japan.
It doesn't cost much to send hair, you can give your hair the second life, someone will be happy to get a wig made of your hair.
So this is how I changed.

If you do a self haircut, you will probably have to go to a hair salon for a proper haircut like I did. The "After" photo was taken right after I got a self haircut. Hair should be cut evenly so they can get hair in about the same length.
Only a form or two to fill and it is not mandatory. You write their address (you can see it at a rather bottom part of the first page of the website), put the hair in an envelope and send. Done!
